Federal law prohibits Medicare providers and suppliers from billing QMB beneficiaries for Medicare Part A or Part B deductibles, coinsurance and copayments for Medicare-covered items and services.

This protection applies even when the provider does not participate in Alabama Medicaid.

Apply even if your income is close to the screening limit. Alabama Medicaid must calculate countable income and make the official eligibility decision.

Approval may stop the monthly Part B deduction, provide Medicare Part D Extra Help and, under QMB, protect you from Medicare-covered deductibles, coinsurance and copayments.

Apply through an official Alabama channel

Complete and sign Form 211, submit it through a current official route and keep proof. The Alabama Medicaid Agency determines eligibility and the approved coverage period.
